Enrollment Documents

  1. Birth Certificate – If parent/guardian does not have one upon entry, the parent may complete the state from, submit a check or money order, and address to the school.  This will suffice for enrollment. To obtain a form:http://chfs.ky.gov/dph/vital/birthcert.htm 
  2. Current Immunization Certificate- Required for Preschool and Kindergarten students upon initial enrollment.  Students enrolling in the District from other counties in Kentucky are required to have a current immunization certificate upon enrollment.  Students enrolling from out-of-state will have two (2) weeks to provide immunization information on a Kentucky form.  
  3. Each student shall be immunized against diptheria, tetanus, poliomyelitis, pertussis, measles, mumps, varicella, hepatitis B and haemophilis influenza disease according with testing and immunization schedules established by regulations of the Cabinet for Health Services.

    The following exceptions apply to testing or immunization requirements:

    Certificate of Medical Exemption:  The student’s physician must write a statement that the child has a certain specific health/physical condition, which are, recognized contraindications to the administration of one or more of the required vaccines.    

    Certificate of Religious Exemption: The parent must submit a written sworn statement objection to the immunization of the student on religious grounds.  

  4. Physical/Preventive Health Care Examination for INITIAL ENTRY is required within one (1) year prior to the child’s initial enrollment to school.  A second examination shall be required within one (1) year prior to enrollment in the SIXTH GRADE SIXTH GRADE or initial enrollment to school.
  5. EYE EXAM  is required by January 1 of the first year that a three (3), four (4), five (5) or six (6) year old child is enrolled in a public school, preschool or Head Start program.
  6. Evidence of a DENTAL EXAMINATION OR SCREENING required by January 1 of the first year that a five (5) or six (6) year old is enrolled.
