Medication Administration by school employee or by student requires a completed MEDICATION AUTHORIZATION FORM to be on file for each school year.
Policy Overview:

Parent signature for over-the-counter will allow the medication to be given for 3 days. If the over-the-counter medication has a physician signature then the medication can be given throughout the school year.

Prescription medication requires a parent AND physician signature.

All medication MUST be brought to office by the parent to be signed in.

All medication will be destroyed once expired or after the end of the school year if not picked up to ensure the safety of all students.

All medication must be brought to school in the original container. Labels that have been altered or received in alternate container will not be accepted.

Medications can not be repackaged for field trips, so a new bottle with an adequate amount of medication for the trip will need to be brought to the school with a current pharmacy label.
Asthma:
Management of asthma by either the student or school employee requires a completed asthma plan each year.
Seizures, Diabetes, Life-Threatening Allergies (food, insects, Latex, other)
