Driver Permit Information



School Compliance Verification: KRS 159.051

At the initial application for an original Kentucky learner’s permit or transfer license only, a 16-17 year old should submit to the

local Circuit Court Clerk’s Office, one of the following documents: (1) proof of graduation from HS, or (2) proof of GED completion,

or (3) a signed and *sealed School Compliance Verification: KRS 159.051 (obtained from the student’s school district of residence).


  • Driver Permit (yellow) Forms are available for pick up in the front office or can be printed from this link

    • Complete “Student Information” section (top portion of form).  

    • Last 4 digits of SS# must be included before form can be processed. 

    • Parent signature is not required to process form but is required before taking the test.

    • Drop off form in the front office.  You will be contacted when form is ready. 

    • Permit forms cannot be processed “while you wait”, no exceptions.   Please allow 24 - 48 hours to complete the process before scheduling your test appointment at the courthouse.


  • Grade and Attendance requirements (per State Law) for eligibility and approval are PER SEMESTER as follows:

    • No more than 8 unexcused absences 

    • No more than 2 - F’s

    • Students must be compliant each semester for the duration of their high school career.

    • Students not in compliance are reported to the State and will have their permit/license revoked until grades/attendance return within State guidelines.
